FOSTER & ADOPTION CASE MANAGER


REPORTS TO: TapKard Inc., dba. Rekindle Treatment Director

QUALIFICATIONS:

QUALIFICATIONS:

1. Must be 22 years of age or older.

2. Must have one of the following:

a. A master’s degree from an accredited college or university; or

b. A bachelor’s degree in a human services field.

3. Must be eligible to hold Child Placing Staff Status according to DFPS standards.

4. At least 2-5 years conducting child placement or case management activities in a child placing agency, group residential operation, or as a conservatorship caseworker or foster adoptive home developer with DFPS.  The experience must be in conducting assessments, service planning, or case management duties.


Primary Purpose: The Foster and Adoption Case Manager is responsible for case management functions that include direct services to foster families and youth, maintenance of records required for compliance with all licensing regulations, and communication with kinship, foster, and adoptive families to ensure that the needs of the children in care are met.

GENERAL RESPONSIBLITIES AND DUTIES


1. Process intake and admission documents for children in care.

2. Travel to kinship, foster, or adoptive homes to conduct a minimum of once monthly home inspections to follow state requirements and Hope Haven policies and procedures.

3. Obtain all necessary documentation from foster families.

4. Monitor and verify that children’s needs are being met in areas of safety, medical care, mental health care, and education.

5. Contact and meet with children in care in compliance with State standards on a regular and scheduled basis.

6. Attend court and State meetings as needed.

7. Prepare discharge plans and manage service transitions.

8. Complete documentation and prepare reports necessary to meet State requirements and Hope Haven quality standards.

9. Identify, develop, and maintain positive relationships with community resources and members of each child’s team.

10. Participate with CPA leadership and staff in training potential foster and adoptive parents to appropriately care for children with trauma backgrounds in compliance with pre-licensing and ongoing requirements.

11. Maintain and monitor case files and all case management paperwork in a complete and timely manner.

12. Participate in treatment planning and staffing for each child in case load.

13. Work with the child placement leadership team to determine whether potential foster and adoptive families are appropriate to serve as placements through Hope Haven of East Texas’ child placement agency.

14. Work with the child placement leadership team to match foster and/or adoptive families with children waiting for placement.

15. Anticipate and maintain records necessary to track and report performance objectives.

16. Maintain communication with DFPS and SSCC representatives as needed.

17. Attend and participate in staff meetings as required.

18. Perform general office tasks as required.

19. Participate in all required training and stay abreast of professional development in the field to enhance knowledge and skills.

20. Perform other assignments and duties as needed.


POSITION REQUIREMENTS, KNOWLEDGE, SKILLS AND ABILITIES


1. Requires basic understanding of a field of knowledge that involves the ability to define problems, collect data, establish facts, and draw conclusions dealing with abstract variables, knowledge normally associated with attainment of bachelor’s degree with a major in a human service-related field.

2. Familiarity with the Texas child welfare system, foster care, and adoptive placement programs, with specific knowledge of the Texas Department of Family and Protective Services Minimum Standards for Child Placing Agencies

3. Ability to work independently with little supervision.

4. Ability to work successfully as part of a team to achieve strategic results

5. Excellent communication and relational skills to effectively communicate the mission and vision of the organization and the needs of children in foster care

6. Ability to motivate listeners appropriately and effectively

7. Excellent verbal and written communication skills

8. Ability to exercise patience, good judgment, and confidentiality; to observe and make accurate assessments of situations with available information.

9. Ability to establish and maintain effective working relationships with children, families, other employees, and the public.

10. Acknowledge the unique Statement of Faith mission of Hope Haven of East Texas and respond appropriately to the cultural differences among the agency’s service population.


This position requires the positive completion of a 90-day probationary period. During this time, the agency and employee will decide if the professional relationship is cohesive. At any time during the 90-day probationary period, either the agency or employee may terminate the relationship for any reason.
